What is the correct form of possessive determiner in this case, Balkan or Balkans? Google results are not helping much with this, it doesn't seem consistent.

When using a proper noun as a modifier, we most typically do not change its form (cf. a Shakespeare play, a Beatles record, etc.).
Thus, the most logical choice would be e.g. a Balkans economic forecast to mean 'an economic forecast relating to the Balkans'.
